Our Fast Track Departnent is reserved for urgent, acute symptoms that you are currently experiencing. If your concern involves an ongoing or more complex medical issue, you will need to schedule a visit with your primary care provider
Does Fast Track offer Well visits?
No. A Physical must be completed by a primary care provider.
Do you accept outside scripts for labs and images?
We do not accept external scripts for lab work or imaging. If you are unsure where to have these services performed, please ask the front desk for assistance. They can provide you with a list of approved locations.
If you would like to have your lab work or imaging done in our office, you will need to schedule an appointment with one of our providers. During your visit, the provider will determine whether in-house testing is appropriate and place any necessary orders accordingly.

Can I be seen by Internal Medicine with an active Workers Compensation Case?
An active workers compensation case can only see the providers the employer approves of. If the patient is in pain, they can be seen through the Fast Track, and the employer will request the medical records of the visit.
What are the pediatric specialties we currently offer?
Our pediatric specialty department offers Endocrinology, Gastroenterology, Nephrology, Neurology, and Urology.
Do I have to bring my insurance card to my appointment?

Can I obtain a copy of my child's physical forms for school or camp?
Yes, if your child has had a well visit with one of our providers within the past year, we can assist you with this. Our medical staff will ensure all necessary paperwork is completed and that your child is up to date on their immunizations. Please note that this process may take up to 10 business days from the time of your request.
Can my childs primary care provider fill out Sports forms for school?
Yes, if your child is a primary care patient at our office and is up to date on their well visits, our providers can assist with this. Please submit the form you received from the school, and ensure that the first page is filled out by the parent. The process may take up to 10 business days from the time of your request
Can I schedule an appointment without vaccine records or medical records?
A patient can be scheduled for an appointment without prior medical records. However, if the appointment is for a well visit and we do not receive vaccine records at least 48 hours in advance, we cannot guarantee that the patient will receive the necessary vaccines during the visit
Can I schedule an appointment while I'm in the process of transferring my care to CHEMED?
If you are feeling unwell and need to see a provider, you can schedule a sick visit. If you require a well visit but are unable to transfer your records in time, you can still schedule an appointment. However, before booking, we will need the date of your last well visit. Additionally, if applicable, please bring a list of any medications you are currently taking.
That said, it is important to make every effort to have your records transferred before your appointment
How long does it take to receive medical records from another practice?
Medical offices are legally required to send records within 30 days. If you need your records sooner, please contact the office responsible for your records and request an expedited process.
Can the doctor fill out forms I need for work?
Yes, you must schedule an appointment with one of our providers to have your work forms completed. Please note that forms can only be filled out if you are up to date on your well visits. If you are not up to date, you a can schedule a well visit and have forms filled out by that visit.

A patient is only required to complete a release form when requesting their full health record to be sent to their personal account, transferring out of or into CHEMED, or accessing their Psychiatry or Behavioral Health records. If a patient is requesting a specific document, such as a lab result, a release form is not required.
Do you offer Same Day medication refills?
It is important to keep track of when your medication will run out, as we cannot guarantee same-day refill requests. To avoid any issues, be sure to stay on top of scheduling follow-ups with your primary care doctor. Our medical staff has up to 72 business hours to complete your refill request. Fast-Track cannot refill medications that were not prescribed by one of their providers.
What should I bring when picking up a prescription?
If you have insurance, please bring your insurance card with you. If you are picking up a controlled substance, a valid photo ID is required. If you are picking up medication for someone else, you must be able to provide their legal name and date of birth to ensure we can assist you properly.

If my insurance requires a referral and I have not seen any medical provider in CHEMED within a year can i come to Fast track to get a referral?
Referrals should come from your primary care doctor to ensure proper follow-up and continuity of care. In urgent situations, Fast Track may provide a one-time referral to a specialist. However, you will need to see your PCP afterward to coordinate ongoing care.
If I have been seen at CHEMED Internal Medicine within the year, but I have not been seen for this specific issue, can I still get a referral?
For any acute issue we can give you a referral without being seen by a provider if youve been seen in our medical department within the year For anything bigger, you must schedule an appointment with a Provider to discuss your concerns and they will decide if a referral is necessary. Once we have the referral on file we can help you get scheduled for an appointment
If I have been seen ONLY at CHEMED SPECIALTY departments, can I still get a referral?
No. The patient must establish care with a PCP in Internal Medicine or pediatrics for us to issue a referral

How can I schedule a vaccine appointment for my child?

Can my child receive vaccines at CHEMED if they are not a CHEMED primary care patient?
No. Our vaccines are reserved for CHEMED patients.
Can I get a TDAP or PPD shot done at CHEMED?
Our Women's Health department offers the TDAP vaccine to all new and existing patients during pregnancy. We also host a walk-in TDAP clinic once a month.
If you are a CHEMED primary care patient and up to date on your well visits, you can call our office to schedule a vaccine visit for TDAP or PPD. However, if you need a PPD for work-related forms, you must schedule an appointment with a doctor to complete the required paperwork.
Do you offer Walk-ins for Vaccines?
We do not offer walk in for vaccines. Vaccine appointments must be scheduled in advance so we can ensure we have the vaccine in stock for you. Our vaccine schedule is open for CHEMED patients ONLY on Wednesday from 9am-1pm and Thursday from 1pm-4pm and 6pm-7pm.
