Do you offer WIC appointments?
We do not offer WIC appointments. If WIC is requesting the patient’s height and weight, we can print out the records from the last visit and that will include the patient’s height and weight. A doctor will see a patient for the height and weight if it is correlated with a medical concern. If you need a WIC form completed, the patient must be up to date on Well visits. Completing the WIC form is a process that can take 7-10 to complete from the time of request.
Do the providers in Fast Track order blood work if I need?
Our Fast Track Department is reserved for urgent, acute symptoms that you are currently experiencing. If your concern involves an ongoing or more complex medical issue, you will need to schedule a visit with your primary care provider.

There are Financial assistance Representatives available to help you in the Women’s Health department, Pediatrics, in Fast Track/ Internal Medicine.

How many days do I have to complete The Slide?
There is no time frame to complete the slide anymore. A receptionist will be able to help you.

Can I obtain a copy of my child's physical forms for school or camp?
Yes, if your child has had a well visit with one of our providers within the past year, we can assist you with this. Our medical staff will ensure all necessary paperwork is completed and that your child is up to date on their immunizations. Please note that this process may take up to 10 business days from the time of your request.
Yes, if your child is a primary care patient at our office and is up to date on their well visits, our providers can assist with this. Please submit the form you received from the school, and ensure that the first page is filled out by the parent. The process may take up to 10 business days from the time of your request.
Yes, you must schedule an appointment with one of our providers to have your work forms completed. Please note that forms can only be filled out if you are up to date on your well visits. If you are not up to date, you can schedule a well visit and have forms filled out by that visit.
Where can I pick up my Medical Records?
Our Medical Records department is located at the CHEMED Madison location in our pediatric department.

How will I know when my medical records are prepared to be picked up?
Once you've filled out your medical release form, our office will prepare your records, and we will be in touch with you once your records are prepared for pick-up.
How can I access my radiology images and report?
We have a radiology website called “Ambra.” We can send this link to patients or providers via email or text. On Ambra, you will be able to view the reports and studies.

Yes, any sort of valid photo identification can be used to obtain medical records.
Can I complete a medical release form electronically?
Yes, if you need to request medical records without visiting our office, you can access our online release form on our website. If you have any difficulty, we can email the release form link directly to the patient’s email address.
Do I need to fill out a medical release form for any record I want sent to me?
A patient is only required to complete a release form when requesting their full health record to be sent to their personal account, transferring out of or into CHEMED, or accessing their Psychiatry or Behavioral Health records. If a patient is requesting a specific document, such as a lab result, a release form is not required.

How do I transfer my prescriptions to the CHEMED Pharmacy?
We can handle the transfer for you. Provide us with your previous pharmacy information and the prescriptions you would like to transfer

If I have never been seen at CHEMED, but the provider listed on my insurance card is a CHEMED provider, can I get a referral without being seen?
No. you are only considered a patient in our office if you see our providers here.
If my insurance requires a referral and I have not seen any medical provider in CHEMED within a year, can I come to Fast track to get a referral?
Referrals should come from your primary care doctor to ensure proper follow-up and continuity of care. In urgent situations, Fast Track may provide a one-time referral to a specialist. However, you will need to see your PCP afterward to coordinate ongoing care.
If I have been seen at CHEMED Internal Medicine within the year, but I have not been seen for this specific issue, can I still get a referral?
If you have been seen at Internal Medicine within the year we can give you a referral for acute issues. For anything bigger, you must schedule an appointment with your provider to discuss if a referral is necessary. Once we have a referral on file, We can help you get scheduled for an appointment.

How can I schedule a vaccine appointment for my child?
