What is an FQHC?
FQHC is an acronym for Federally Qualified Health Center—a community health center that has been designated by the federal government to provide quality health care services to everyone, regardless of ability to pay.
What Should I bring to my first appointment?
For your first appointment at CHEMED, whether it be for a sick or well visit, please bring any copies of old health records, lab reports, or immunizations that you may have. If you do not have this information, you can fill out a waiver at CHEMED to have the documents sent from the physician you previously went to.
If you are uninsured or underinsured and will be applying for the sliding fee scale please bring either two consecutive pay stubs or a tax return as proof of income, as well as a utility bill or bank statement as proof of residence. You will also need proof of identification such as a driver’s license or a birth certificate.
How long do I need to fast before having my blood test?
If you are having a blood test that requires fasting, please do not eat or drink anything after midnight. CHEMED’s laboratory has appointments starting at 9am.
