Integration
Better Collaboration.
Better Outcomes.
CHEMED's unique healthcare model puts the patient's designated primary care doctor at the forefront of their care plan. Maintaining a personal relationship with the patient, the primary care doctor works collaboratively with CHEMED's in-house team and affiliated specialists and professionals.

The entire team at CHEMED inputs directly to the patient's healthcare file, which can be shared when referring patients to specialty providers for diagnoses or treatments. Evaluating the entire patient via a single integrated module ensures that treatments, diagnoses, meds and lifestyle regimens take into account every facet of the patient's health.
