CHEMED´s care management program uses an innovative approach to advance the quality of care provided to our patients. The Care Management team consists of primary care providers, nurses, referral coordinators, care coordinators, and a designated case manager from the LRRC. Working collaboratively, this team aims to personalize and coordinate care as well as improve health outcomes.
